The Swan Surgery – Caring for you by knowing you

The Swan Surgery is a modern, accessible and friendly practice. We are a dedicated team of health professionals committed to provide you with the highest quality of care.

We offer a comprehensive range of services from a modern premises with an onsite pharmacy situated  close to the centre of town. We have close links with Petersfield Community Hospital which is adjacent to the surgery

We invest time in getting to know our patients and being able to provide continuity of care, this way we can take a more holistic approach to our patient’s health and wellbeing.

Swan Surgery has suitable access for disabled patients, and all the patient areas including the waiting room, the consulting rooms and the toilets, have wheelchair access.

Designated disabled parking spaces are located nearest to the entrance of the building.

Health Promotion Event:

On Saturday morning, 10th May  we are holding  a free health promotion event in collaboration with our Patient Participation Group (PPG). The event is focused on Asthma, Hay Fever, Sun Damage & Eczema and booking is required. To book your place or for more information please call: 01730 264546.

Please note we will be closed all day on Monday 5th May and will re-open Tuesday 6th May at 8.30am. If you need medical advice please call the out of hours GP service on 111 and for medical emergencies please call 999.  


Ongoing Care Clinics

We have revised the way we manage Ongoing/Chronic Condictions. These condictions include high blood pressure, coronary heart disease, diabetes, asthma and COPD. You will be seen by a Nurse or Health Care Assistants here at the Surgery, who will review your condictions in an appointment and give advice and guidence if needed. The Nurse or Health Care Assistant will then make your next appointment for you, if any problems appear we can refer back to your own GP. If any problems or concerns please contact us.



We have been notified that some patients are reporting receipt of an email notifying them of blood test results that indicate that the patient has cancer.  The email appears to be from NICE.  This email is a scam, please go to for full information.

Announcements –

We are undergoing a major change on Wednesday 30th April when we move to a new I.T. system for our clinical records. During that day GP’s and staff will be getting to grips with a very different way of working so please be patient with us. If your need is not clinically urgent on April 30th it would really help us if you did not use the surgery that day.
