EMIS Access for patients

How to register :

 

To become a new patient here at the Swan Surgery you are required to make an appointment with one of our Health Care Assistants for a health check up and bring along the following items:

 

  • Completed registration form (LINK)
  • Completed GMS1 form (LINK)
  • Medical card (if you have one)
  • Photo I.D (photo card driving licence or passport)
  • Proof of address (utility/phone bill or letter)
  • Sample of urine (any type of bottle or container will suffice as long as it has been sterilised. Chemists also sell sample bottles at around 50p)
  • It is important we have your previous postcode

 

 

Overseas patients :

 

If as a new patient you have recently arrived from overseas and this is your first registration with a UK GP practice you will be required to initially complete a ‘Confidential Patient Registration application form’  (please click here to download) and bring along the following items:

 

  • Any medical information you may have
  • Immunisation record or report of recent chest x-ray
  • Letter from the Health Authorities (PPSA) if you have one

 

 

 

Temporary patients :

 

If you are in the area and need care you will need to register as a temporary patient and complete the following:

 

  • GMS3 form (please collect this from reception)

 

If you are a temporary patient from overseas and from Europe you will need to provide an EHIC (European Health Insurance card) you will also need to complete a ‘Confidential Patient Registration application form’ (please click here to download). Please be aware that you may be charged for treatment needed if you do not have the correct information or are not eligible for NHS care.
