To become a new patient here at the Swan Surgery you first need to check if you are in our catchment area, shown below. Please enter your post code into the box provided.
If you are out of our catchment area but would still like to join our practice, please contact the surgery to find out more details. Please note that we cannot provide out of area patients with home visits.
If you are eligible to register here, you will need to complete a Patient Registration form (link below) this can be emailed to us at firstname.lastname@example.org. If you do not have the facility to email your registration form, you can bring this to reception or drop it into our post-box (if you are dropping off your form in person please do bring ID with you if you have requested an online account).
Once we have received your registration information and if you are requesting an online access account, you will receive a phone call from us in order to verify your identity (please note if we are unable to verify your information over the phone you may be asked to bring in a form of photographic ID in person).
If you take regular medication please ensure you have enough supply for 1 month when you come to register, this will give a smoother transition and allow enough time for your previous practice to process your deduction, and will give us enough time to process the new registration during this transition period.
